For most Scouting events lasting up to 72 hours, Part A, Part B1, and Part B2 are required to be submitted. Please include a photocopy of both sides of your insurance card, if applicable.
For Scouting events lasting longer than 72 hours such as Summer or Winter Camp, an annual pre-participation physical by a certified and licensed doctor (MD, DO), nurse practitioner, or physician assistant is required as documented on Part C.
To ensure that everyone can have fun and remain safe on all Scout outings, Troop 183 is required to maintain completed Annual Health and Medical forms for all Scouts and Adults attending events.
